Connect with your provider from home via Telehealth
Our patients, community, staff, and clinicians are our number one priority, and their safety is always the most important factor we consider, especially during this time of COVID-19. At our practice, we’re changing the patient delivery system to provide the majority of visits by Telehealth. We want to ensure that we can continue taking care of our patients, but in a way that keeps them and our staff safe during this public health crisis.
Telehealth means you can connect with a provider remotely, right from the comfort of your home. Check our website for details of our Telehealth services. For appointments, Mountain Health Center patients can call the practice directly at 802-453-5028.
There is access to face-to-face primary care visits at Mountain Health Center, but the majority will be a Telehealth visit. Thank you for your confidence in Mountain Health Center as we continue to provide care to our community during this difficult time.
We are committed to keeping our patients and community safe from coronavirus. If you have a fever, cough, or shortness of breath or are concerned about possible exposure to coronavirus, please call us at 802-453-5028 as we are happy to discuss over the phone and figure out the appropriate next steps.

Community Health Needs Assessment – Your input makes a BIG impact!
