We are excited to announce our School-Based Health Center, a collaboration with MAUSD to improve the health and well-being of our school communities. Nurse practitioners and physicians from Mountain Health will be onsite at Mt. Abe on Wednesdays to provide routine and urgent care to students and staff. Visits will be conducted in Mountain Health’s mobile care unit and can be scheduled through the Mt. Abe school nurses. Students and staff who are not Mountain Health patients can also be seen through the School-Based Health Center and do not need to change their primary care provider. If you have a student at Mt Abe and would like them to benefit from this service over the coming school year, please sign the consent forms (available at this link) and return them to the health office. Visit mausdhealthcenter.com for more information. We are hoping to shape and grow this program to meet our schools’ needs in the coming years; feedback is welcome!
Connect with your provider from home via Telehealth
Our patients, community, staff, and clinicians are our number one priority, and their safety is always the most important factor we consider, especially during this time of COVID-19. At our practice, we’re changing the patient delivery system to provide the majority of visits by Telehealth. We want to ensure that we can continue taking care of our patients, but in a way that keeps them and our staff safe during this public health crisis.
Telehealth means you can connect with a provider remotely, right from the comfort of your home. Check our website for details of our Telehealth services. For appointments, Mountain Health Center patients can call the practice directly at 802-453-5028.
There is access to face-to-face primary care visits at Mountain Health Center, but the majority will be a Telehealth visit. Thank you for your confidence in Mountain Health Center as we continue to provide care to our community during this difficult time.
