After Hours/Holidays/Weekend Care:
A provider is always available after hours. We share in a call schedule with the providers of Middlebury Family Health, and the on-call service is managed by Porter Medical Center. Patients needing care outside of regular hours will be contacted by the on-call provider if necessary or may be referred to the emergency department at Porter Hospital if they require care which cannot be provided in the office.
For care outside of regular business hours, call us at (802) 453-5028 and choose the option to be directed to the on-call service.
Mountain Health Center offers translation services for our patients as well as Telecommunications Relay Service (TRS) and American Sign Language.
Inpatient Hospital Care:
Patients requiring inpatient hospital care will be admitted and cared for by the hospitalist (a hospital-employed physician who will oversee care during hospitalization) who may consult with the on-call provider, as needed to help coordinate care. Upon discharge from the hospital, your care will be coordinated with your provider through either written or spoken contact with the hospitalist.
Porter’s on-call pediatrician admits to the nursery and we admit to Helen Porter Nursing Home. Our providers will care for their patients if they are living in a local home or nursing home. Home visits may be scheduled if you are too ill to travel to our office for scheduled care or acute visits.
