About Our Team
At Mountain Health Center, each patient has a designated provider of choice to oversee all care. We make referrals to specialists and coordinate care as needed. We also work with home health services to provide in-home support to our patients when needed.
Mountain Health Center is a medical home where people can engage in coordinated and comprehensive health care in a comfortable, caring and relaxed environment. Comprehensive care plans which include patient generated goals and specialty referrals, as well as coordination of care, are integral to our practice. We take care of you when you are ill and work with you to achieve health, recognizing that your health care demands your involvement.
We now have a Certified Application Counselor (CAC) to help our patients and community members apply for qualified health plans, including Medicaid, through Vermont Health Connect.

Marian Bouchard, MD – Marian has been with Mountain Health Center since 1993, practicing full spectrum family care, often seeing several generations of a given family. She enjoys the relationships that develop over time, and feels that trust is vital to a positive healing environment. Marian went to McGill University Medical School, in Montreal, Quebec, and completed a family practice residency in Rochester, NY. Mountain Health Center was her first job out of residency, and for the first 9 years of practice, Marian included obstetrics in her practice. That changed when Marian had her own two children. Marian enjoys cycling, skiing, playing family soccer, spending time in and on the water, and in nature with her family. She is interested in wildcrafting, yoga and when time allows, loves to garden. She is very interested in how health is connected to one’s relationship with food, and Marian loves to cook with this in mind. Marian is board certified in both Family Medicine (American Board of Family Medicine) and Integrative Medicine (American Board of Integrative Holistic Medicine). Currently NOT accepting new patients.
Renee Bratspis, MD is joining the Mountain Health Center team and will be available for appointments beginning November 1, 2016. Renee grew up in Monkton and attended Mt. Abe Union High School. She attended undergrad at McGill University and went on to study medicine at the University of Vermont. Renee completed her residency in family medicine and a fellowship in obstetrics and gynecology in Ventura, California. She is excited to be moving back to provide full spectrum family medicine care in her hometown. In her free time Renee enjoys spending time with her family, skiing, hiking and reading.
Ed Clark, MD – Ed is one of the original founders of the Mountain Health Center, which started in 1986. He graduated from UVM Medical School in 1982 and completed a Family Residency Program at UVM in 1985. Now mostly retired, Ed still helps out at MHC and volunteers at the Open Door Clinic. Ed lives in Lincoln (in the house he built) with his wife Suzanne and has two grown daughters. Ed is an avid kayaker, mountain biker, skier and an accomplished carpenter. Currently NOT accepting new patients.
Rachel Edwards, M.S., L.Ac. – local acupuncturist, herbalist, and massage therapist, brings complementary care therapy to the scope of services offered at the Mountain Health Center. She specializes in addiction and behavioral health, and sees her clinical role as a facilitator for each person’s unique capacity for transformation and healing. While every clinical relationship is different, she invites each person to participate more fully in the practice of health through dietary changes, meditation, and exercise. Rachel received her certification in massage therapy from the Vermont Institute of Massage in 1997 and graduated from the Pacific College of Oriental Medicine in 2001. She has been sharing the benefits of Chinese Medicine and massage therapy with the Vermont community ever since. Rachel lives in Lincoln with her husband and three sons. If she’s not enjoying family time in the woodlands and by the rivers, you can find her practicing Tai Chi Chuan out in the fields with the view of Mount Abraham.
Suzanne Germain, ANP – Suzanne is one of the original founders of MHC with her husband Dr. Ed Clark, which started in 1986. She has lived in VT since age 11 (so not quite a native Vermonter). Suzanne graduated from UVM nursing school with an AD in 1976 and a BSN in 1985, and then became a Women’s Health Nurse Practitioner. She just recently completed her masters in 2013 as an Adult Nurse Practitioner. Her work as an NP has taken her all over the world volunteering in Mexico, Nicaragua, Honduras, Peru, Dominican Republic, Haiti, Ethiopia, and locally at the Open Door Clinic. In addition, Suzanne speaks fluent Spanish and French.
Nidhi Gupta, DMD – Nidhi Gupta grew up in India and moved to United States where she received her medical training. She graduated with a Masters Degree in Pharmaceutics from the University of Georgia and completed her training in dentistry at Boston University’s dental school in 2006. After working in Maine for two years at a non-profit dental center, she married and moved to Vermont. Nidhi enjoys working in public health as it gives her the opportunity to provide care to people in need of all kinds of dental services ranging from immediate pain relief to cosmetic dentistry. She enjoys sharing her skills with adults and children of all ages and likes to volunteer for free dental care days. Nidhi believes preventive care is essential and encourages patients to be seen as early as when their first tooth appears. This gives her the opportunity to build trusting relationships with families over time providing information on preventative care along the way. Nidhi enjoys swimming and various outdoor activities, and spending time with her husband and two children.
Amber Jimerson, FNP – Amber joined Mountain Health in 2015 and is now currently expanding her practice. She is a Family Nurse Practitioner who can see the entire age spectrum. Amber received a Bachelors of Arts in Psychology from Bates College, and a Bachelors of Science in Nursing & Masters of Science in Nursing from the University of Rochester. Her practice philosophy includes following the latest evidence-based approaches to care while developing trusting relationships with patients. Amber has spent time working in neuro-medicine, urgent care, and occupational medicine, and is excited to develop a new primary care focus. When not at work, Amber lives and plays locally with her two children, two dogs, and husband. She has a special interest in all things outdoors (sailing, swimming, paddle boarding, skiing, hiking, biking, gardening, and collecting caterpillars and tadpoles with her son).
Frank Provato, MD – Dr. Provato is Board Certified in Internal Medicine and specializes in the treatment of a broad spectrum of acute and chronic illnesses and injuries in patients aged sixteen and older. He has a special interest in the treatment of diabetes, high blood pressure and chronic heart and lung disease. He received his medical degree from the State University of New York Upstate Medical University in Syracuse, New York. He completed his post-graduate training in Internal Medicine at Albany Medical Center in Albany, New York. He has over forty years of experience in clinical practice and medical administration. In addition to seeing patients, he also serves as Medical Director of Mountain Health Center. A native of New York City, Dr. Provato has been in Vermont for almost two decades serving patients in both urban and rural settings. In his spare time, he enjoys fishing, hunting, reading, listening to music and spending time with his family. Dr. Provato is currently accepting new patients.
John Viskup, DMD – Dr. John Viskup grew up in central New York State and graduated from the University of Pennsylvania School of Dental Medicine. He served his residency at the Mary Fletcher Hospital in Burlington, Vermont and after practicing briefly in New York City returned to Vermont purchasing a practice in Vergennes. He also started a dental practice in Waitsfield, Vermont, which recently was severely damaged by Hurricane Irene. Dr. Viskup has seen dramatic changes in the field of dentistry during his nearly 50 years of experience. He feels very fortunate to not only have the opportunity to help people have a beautiful smile, but encourages them to be aware of the fact that a healthy mouth is the first step to overall good general health and happiness.
Jeff Wulfman, MD joined Mountain Health Center in 2013. He practices full-spectrum Family Medicine covering all areas of health and all ages from birth to elderly. The original foundations of Jeff’s general philosophy honor all aspects of evidence-based medicine including scientific information, clinical experience and patient preferences/values. Jeff takes an open-minded, carefully considered, common-sense approach to healthcare, combining the best of both standard allopathic medicine and integrative/alternative/functional medicine approaches. He sees the body as an ecosystem, not just a chemistry set, and honors that each person’s situation is unique and is best served by individualized care with decisions made as a team- clinician and patient- working together with mutual respect. Jeff is board certified in both Family Medicine (American Board of Family Medicine) and Integrative Medicine (American Board of Integrative Holistic Medicine).
